Once the first climber reaches the top - a safety rope should be placed on the top of the ladder to an anchor point.  This will prevent the hook from becoming dislodged  as others are climbing over the in place hook. If at any time the climber feels that it is unsafe, they should stop climbing and climb back to the starting location.
